By submitting this application form I, the student/parent/guardian hereby confirms the acceptance of the terms and conditions of the Admission Agreement (below) with Horizon International.

Admission Agreement: I have read and understood policies and conditions governing enrollment and admissions to Horizon International(HI) below and accept it before student enrollment. 1. Student/Parent/Guardian agrees to enroll the student at HI for the selected programs in this form. 2. Student/Parent/Guardian agrees to submit 'Student Withdrawal/Subject add-drop form' at least 1 month before student's withdrawal. 3. HI may remove the student from the Academy for reasons related to misconduct,long term absence or non-payment of fees. 4. HI reserves the right to revise any timings listed according to overall applicant choices. 5. In the event of any emergency, where the parent/guardian cannot be contacted, HI may authorize the medical examination of a student, the calling of further medical or specialist advice, or removal of the child to a Medical facility, all expenses thereby incurred will be on the student/parent/guardian’s account. 6. The student/parent/guardian accepts that HI will not be responsible for any personal injury or liable for any loss or damage of any kind whatsoever which the student may sustain at any time either within the premises or elsewhere that is not attributable to the negligence of the Academy or its employees. 7. The student/parent/guardian also authorize HI to use image of the students for any promotional activities so long as the image forms part of the Academy’s activities. 8. Student/Parent/Guardian accepts that any tuition fees paid to HI are non-refundable. 9. Student/Parent/Gardian agrees to pay tuition fees on or before 10th of each month. 10. HI has the right to charge a fine of MRF 50.00 if the monthly fees are not paid by the 10th of each month. 11. Students may not be allowed to attend classes unless all fees payable are paid. 12. Until 'Student withdrawal/Subject add-drop form' is submitted, student shall be considered as an active student and monthly fees shall be charged. I agree to the above terms and conditions of Horizon International. I also declare that the information provided in this application form reflects the truth. I acknowledge that Horizon International reserves the right to alte or reverses any decision regarding admission or enrolment made on the basis of incorrect or incomplete information.
